That's why the Saint Agnes Hospitalist Group, in conjunction with the multidisciplinary team of caregivers across the Medical Center, are the preferred providers of hospitalist services at Saint Agnes. We are dedicated to providing quality care 24 hours a day, seven days a week, to patients – like you. This group of physicians specializes in caring for seriously and critically ill patients, which is why your primary care physician has arranged for the Saint Agnes Hospitalist Group to provide your medical care during your hospital stay.

Our hospitalists are board-certified in internal medicine and devote 100 percent of their time to a hospital-based medical practice. Hospitalists know every specialist and department within the Medical Center. They ensure that a member of the team is available to meet your round-the-clock needs. This allows for a more timely response to urgent problems that may arise. Hospitalists assist you through a smooth and speedy recovery process by following up on tests and adjusting your treatment regiment throughout the day based on those test results. Since our hospitalists work as a team, it is likely that more than one of them will be involved with your hospital care.

High quality care is provided by a team of hospital-based doctors with experience, expertise and compassion.

Essential to our team are the highly experienced nurses working as Case Managers. They coordinate communications with all members of the care team to expedite treatment plans, communicate with patients and families, and plan for patient needs after hospital discharge. They can arrange for home health care, skilled nursing or rehabilitation placement, or hospice care, as ordered by your physician. Case Managers are an essential component that enable the hospitalist program to consistently produce sustainable results in efficiency, quality of care, and patient satisfaction. 

Working with a hospitalist team is a win-win proposition for both patients and primary care physicians. Using the hospitalist team enables your primary care physician to be more available when you are in need of outpatient care. Your primary care physician will be notified of your admission to the hospital and may be consulted about important issues related to your inpatient care. He or she will also be notified about your final diagnosis and of the medications and follow-up plans prescribed at the time of your discharge. After you are released from the hospital, your primary care physician will resume responsibility for your medical care.
