Birth Certificates

To request a copy of an official birth certificate pertaining to a child born within the last two years, please contact the Division of Vital Statistics, (559) 600-3310.

Please note: Saint Agnes Medical Center does not retain birth certificates within the hospital. Please do not contact the hospital for a copy of a birth certificate.

If you would like to make changes to a child's birth certificate, including naming, please contact our Saint Agnes Birth Certificate Technician at (559) 450-3785.

To request a copy of an official birth certificate pertaining to a child, age 2 years or older, please contact the Fresno County Recorder office directly, (559) 600-3476.

*Requests may not be submitted via fax, phone or email. Requests can only be made in person or by mail.
