In order to be an on-campus instructor to students at Saint Agnes Medical Center, please review the important documents listed below. Then, complete and sign the required documents and email them to nursing.education@samc.com.

Required documents ­- Please complete and email to nursing.education.com no less than 2 weeks prior to rotation start date.

1. Orientation Validation Quiz
2. Pages 2-4 of Instructor Clearance Packet (rev. 07.2025) — Keep page 1 for your records
3. Student Processing Roster (clearance information for students) for every clinical group or individual students
4. Instructor Processing Roster (clearance information for instructor) to be done and on file once per calendar year.
