At the heart of Saint Agnes Medical Center lies a deep commitment to our community. We believe in the power of collective action, where every effort contributes to the greater good. We are committed to meeting the diverse needs of our growing population and to healing the whole person—body, mind and spirit.

Saint Agnes Community Health and Well-Being (CHWB) is focused on addressing the following areas:

Access to Care

Alongside the Saint Agnes Graduate Medical Education Program (GME) and the Fresno County Department of Public Health (FCDPH), Community Health and Well-Being launched its Rural Mobile Health program. This program provides medical services at no cost to agricultural workers and residents living in rural areas of Fresno County.

Social Determinants of Health

Saint Agnes' CHWB strategy promotes optimal health for people experiencing poverty in the community. We serve by connecting social and clinical care, addressing social needs. Many patients of Saint Agnes are unable to follow their medical care plans because of non-medical barriers such as: lack of financial resources, insurance, or transit. The Saint Agnes Social Care Hub pairs Community Health Workers with patients and their families to connect them to appropriate resources.

Provider Shortages

Saint Agnes is a teaching hospital where physicians, nurses, nursing students, and non-clinical health professionals can grow and train in an environment of diversity, equity and inclusivity. Our hospital welcomes everyone through its doors—regardless of background—and encourages new ways of thinking, behaving, and caring.

Increasing Healthy Food Access

To address food insecurity in the Valley and help find environmental solutions, Saint Agnes partners with Food to Share. Through this project, those living in underserved communities in Fresno County now have access to nutritious, unused food that would otherwise be thrown away and end up in a landfill.

Safe Neighborhoods

The Transforming Communities Initiative (TCI) is an innovate funding and technical assistance initiative geared toward advancing health and racial equity among communities experiencing high poverty and other vulnerabilities. In partnership with Fresno Housing, Saint Agnes and Trinity Health are working within the 93706 community of West Fresno and collaborating with residents living in the California Avenue neighborhoods to shift toward a community-centered public safety model, designed to ensure the safety of ALL Fresno residents.
