At Saint Agnes Medical Center, we strive to exceed the expectations of our patients and families, and we are blessed with individuals who are truly passionate about the work they do. If you or a loved one felt that one employee, clinical or non-clinical, touched your spirit while in the hospital, we want to hear about it!

There are now three ways to recognize our colleagues.

There are now THREE types of DAISY Awards available at Saint Agnes: DAISY Award for Extraordinary Nurses, the DAISY Team Award, and the Nurse Leader Award. Click the accordions below to learn more about each type.

This award honors nurses who demonstrate a commitment to excellence and our Core Values. These extraordinary nurses go above and beyond their normal call of duty by demonstrating their expertise and compassionate care with every patient, always.

All DAISY Award nominees must represent the following: 

  • Reverence: I connect with compassion and courtesy.
  • Commitment to Those who are Poor: I notice when others are suffering or struggling.
  • Justice: I build and maintain healthy and trusting relationships.
  • Stewardship: I support others in fulfilling our mission. I own every problem and seek to find resolution.
  • Integrity: I put people at the center of all I do by being present and attentive.
  • Safety: I provide an environment that prevents harm and nurtures healing for all.

This award honors collaboration by two or more people that is led by a nurse. This award will be presented to a TEAM of nurses that lives by our core values and, through its teamwork, has had an extraordinary impact on patient care.

Criteria includes:

  • Team must be led by a nurse or a group of nurses
  • Team must be comprised of two or more people
  • Team must exemplify Saint Agnes' Mission and Core Values
  • Team members model collaboration and teamwork that makes a difference in the lives and experience of patients and families

This award honors managers, directors, practice coordinators, educators, chief nursing officers, and preceptors who provide extraordinary leadership by demonstrating compassion to their staff and by providing support, both physically and emotionally.

Criteria includes:

  • Models compassion and exemplary practice
  • Creates and fosters an environment of trust, compassion, respect for professional development and ethical behavior
  • Accessible, available, and responsive to staff
  • Encourages critical thinking and problem solving for individuals and teams
  • Motivates staff with a shared vision and enthusiasm to achieve better outcomes for themselves and for their patients
  • Promotes and enhances Saint Agnes' commitment to serve together in the spirit of the Gospel as a compassionate and transforming healing presence within our communities
  • Demonstrates leadership by promoting and supporting the image of nursing within the organization, the community and the profession

 

GEM Award Logo

The GEM Award recognizes colleagues for Going the Extra Mile to help patients and their families. No matter what the challenge, GEM Award recipients always take initiative, think positively, act professionally and perform above and beyond what is expected of them.

We believe a GEM Award nominee is a colleague who:

  • Lives our healing mission and core values
  • Offers kindness and connection to those we serve
  • Encourages and supports others
  • Maintains a positive attitude and cheerful spirit
  • Delivers excellent patient care

 


Safety Hero Award

The Safety Hero Award is presented to colleagues who live out our Core Value of Safety, for speaking up (see something, say something), stopping the line and being a champion for safety.

A Safety Hero helps keep our facilities safe by making safety a top priority in their daily work and demonstrate one or more of these attributes:

  • Maintain key safety behaviors benefiting patients, colleagues and/or Saint Agnes facilities
  • Play an integral role in a good catch or near miss during a safety event
  • Demonstrate one or more of Trinity Health's Foundations of Safety behaviors:
    • Prepare for the process
    • Manage the task
    • Support the team
    • Communicate clearly
    • Have a questioning attitude
    • Pay attention to detail
