About Central Valley Health Plan


Central Valley Health Plan (“CVHP”) is a State of California licensed health plan (Restricted Knox-Keene) that offers Medicare Advantage plans to residents of Fresno and Madera counties. Central Valley Health Plan is wholly owned by Saint Agnes Medical Center in Fresno, CA. The plan was started as a collaborative effort that combines the strengths of a community hospital and IPA/Medical Group with long-standing commitments to serve the health needs of the local community. CVHP, and Saint Agnes Medical Center, are part of Trinity Health, a leading not-for-profit Catholic health system serving communities across the country with 92 hospitals and hundreds of primary, specialty and continuing care centers in 22 states.

The CVHP is committed to providing high value care to the diverse population in the Central Valley. In support of this commitment, the CVHP mission statement is:

To accomplish this mission, CVHP will:

  • Establish a risk sharing model that incentivizes Participating Providers to meet quality and cost-of-care benchmarks;
  • Leverage expertise from its Partners to support the medical home model;
  • Provide a consistent, patient-centered clinical experience across the Participating Provider network;
  • Spread innovation and best practices across the full continuum of care;
  • Utilize population health and data analytics to assess and optimize patient care.
