The Health Information Management department at Saint Agnes uses a state-of-the-art electronic medical record system. This creates a more secure environment for health information, however, still provides immediate access for physicians to review the patient's medical record. (Note: Hard copies of all medical records are kept secure for a minimum of seven years.)
How do I request copies of my medical records?
Patients can order copies of their medical records by filling out this simple authorization form (spanish) and paying a fee based on the number of pages requested.
Forms and instructions for requesting medical records can be picked up in person at the Health Information Management Department located on the first floor of the hospital's Main Wing, just west of One Main Café. We are also happy to fax you an authorization form. To request a form via fax, please call (559) 450-3327.
Once the authorization form is completed, please mail to the Health Information Management department:

When the authorization process is complete, photo identification must be presented to the Health Information Management prior to release. Records will not be released without proper identification shown. Any additional fees will be collected at that time.
Fees for records:
$ .25 cents per page
$ .50 cents per page if microfilmed
Records may also be mailed to the following if requested:
- Records for continuing care may be mailed to a physician at no charge to the patient.
- Records may be mailed to an attorney and appropriate charges will be collected from the patient.
- Records may be mailed to Social Services/Disability at no charge to the patient.
- Records may be mailed to Insurance Companies, if applicable the Insurance Co. will be billed.
If records are mailed, an invoice will accompany the records and the payment can be mailed with the return envelope.
