HOME CARE SERVICES

In 1970, Saint Agnes Medical Center opened the first home health agency in Fresno. Our goal, then as now, is the provision of quality health care to patients in their own home.

Today, Saint Agnes Home Health offers many skilled intermittent services to homebound patients, often eliminating hospitalization, or bridging the transition of care to the home. If you meet home health criteria (skilled need, homebound status) and have an order from your physician, home health care is covered by Medicare, Medi-Cal and private insurance. Saint Agnes Home Health is available seven days a week, 24 hours a day to meet your intermittent needs at home.

The program provides skilled observation, assessment, and instruction to both patients and families. Our nurses and therapists provide care as ordered by the patient’s physician including:

IV therapy
catheter insertion
wound care
ostomy care
other skilled services

Saint Agnes Home Health offers:

skilled nursing services
physical therapy
medical-social services
occupational therapy
speech therapy
IV therapy
home health aide services
stoma/ostomy care
nutritional and dietary consultation

To meet the changing and complex needs of our patients, Saint Agnes Home Health works with other providers, and can make referrals for:

durable medical equipment
outpatient physical therapy
emergency response system
outpatient speech therapy
hospice care

Advance Health Care Directives

The Patient Self-Determination Act of 1990 requires that all Medicare and Medi-Cal providers inform patients about their right to make decisions about their own health care. This includes their right to accept or refuse medical treatment and their right to prepare Advance Health Care Directives. These directives include documents such as a Power of Attorney for Health Care or an Individual Health Care Instruction. These are legally recognized documents designed to communicate our wishes for medical care when we are unable to decide for ourselves or communicate decisions.
