If you are interested in receiving a copy of an official birth certificate for a child born within the last two years, please contact the Division of Vital Statistics - Fresno County Human Services System or fill out the request form located in the back pocket of your beige "Women and Infant Services" folder (given when you gave birth at Saint Agnes).You can mail this request form or take it to the Division of Vital Statistics in person. The address and phone number are listed below.
